Shohei Ohtani underwent elbow surgery Tuesday morning, agent Ness Balelo announced. The process was done with “greater emphasis on the bigger picture” of his career, Paleo said.

Ohtani, who posted a 3.14 ERA with 167 strikeouts over 132 innings this year, made the last of his 23 starts on Aug. 23 when a UCL tear forced him to hit exclusively for the rest of the season.

“I had a procedure done on my elbow this morning and everything went well,” Ohtani said in a statement on his Instagram account. “Thank you so much for everyone’s prayers and kind words.

“It’s unfortunate that I can’t finish this year on the field, but I’ll be rooting for the boys until the end.

“I’m working as hard as I can and I’ll do everything I can to get back on the diamond stronger than ever.”

“The final plan after consultation with Shohei was to repair the problem in the arm and reinforce the healthy ligament while adding tissue that would allow for longevity of the elbow,” said Dr. Neil Eladrach, who performed the surgery. “I expect him to make a full recovery and be ready to hit on opening day in 2024 without a hitch. Ohtani was ruled out for the season on Sunday due to a lingering oblique problem. He was out of the lineup for 11 games before being placed on the 15-day injured list.

That issue is further complicated by the fact that this is the second major surgery on Ohtani’s right elbow. He underwent Tommy John surgery to repair a tear in the same UCL at the end of the 2018 season. It ruined his 2019 season, and because of arm problems and a pandemic-shortened season, he made just two starts in ’20.

When Ohtani fully returns to the mound in 2021, he continues one of the most impressive three-year stretches in baseball history — on both sides of the ball. From ’21-23, Ohtani crushed 124 home runs and posted the fourth-best OPS+ (161) among hitters with at least 1,000 plate appearances. He struck out 542 batters in 428 1/3 innings, with the second-best ERA+ (152) among starters with at least 400 innings.

Along with his contributions as a two-way player, Ohtani’s 26.3 WAR fan charts the most in all of baseball during his three-year prime. With the surgery sidelining his pitching contributions until at least the 2025 season, it will be a while before we see the two-way phenomenon return to action on both sides of the ball.
